The IGNITE II CC: Engagement, Coordination, Demonstration, and Dissemination

Project Summary This proposal is in response to a Notice of Special Interest (NOSI) entitled “Urgent Competitive Revisions for Research on Critical Threats to Public Health in Large-Scale Population Cohorts” (NOT-HG-21-020) to focus on NHGRI’s interest in supporting research on pressing public health questions that can uniquely be addressed by collaborative efforts of large international cohort studies. Specifically, we propose to coordinate efforts focused on the collection and analysis of data or samples from ancestrally diverse cohorts of individuals participating in the International HundredK+ Cohort Consortium (IHCC). This proposal will concentrate on the engagement of cohorts from limited resource settings/low- and middle-income countries (LRS/LMICs). The IHCC is supported by the administrative secretariat of the Global Genomic Medicine Collaborative (G2MC), a not-for-profit organization established with the goal of bringing together the global community to enable the implementation of genomic medicine to improve individual and population health. A major goal of the G2MC is to engage the global genomic medicine research community in robust scientific exchange of ideas to advance genomic medicine to clinical practice. We propose to enlist the experience and organizational expertise of the Implementing GeNomics In pracTicE Pragmatic Trials Network (IGNITE PTN) Coordinating Center (CC) at Duke University to work with the G2MC as partners with aligned goals. The IHCC is a community consisting of 103 participating cohorts located in 43 countries, with total current sample sizes across all cohorts of > 50 million participants. This proposal aims to utilize the IGNITE PTN and G2MC secretariat expertise to: 1) expand seven recently awarded IHCC projects to LRS/LMIC cohorts so as to increase ancestral and geographical diversity of the projects as well as the standardization of data capture from the cohorts using resources of the IGNITE PTN, IHCC, and NIH (e.g. PhenX Toolkit); 2) improve inclusivity of LRS/LMIC cohorts using surveys and interviews of LRS/LMIC cohort leadership to determine the gaps and identify solutions required to enhance their effectiveness as IHCC members and as research sites; and 3) plan and pilot the development of a new educational platform and establish an “Education Working Group” to engage, inform and mentor faculty and trainees (the “cohort leaders of tomorrow”) from LRS/LMICs who may wish to build novel cohorts or enhance existing ones. Our track record of collaborative genomic medicine research, program management, and nationally recognized expertise in coordinating large networks uniquely positions us to facilitate the IHCCs strategic goals for cross-cohort communications, scientific program development, capacity building through education and knowledge dissemination, and a robust and interactive web platform.
