This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. Sickle cell disease results from an Adenine(A) to Thymine (T) substitution in codon 6 of the beta globin gene which specifies the insertion of valine in place of glutamic acid. This disease occurs in 1:900 at risk individuals (African Americans, Hispanics). It results in polymerization of the hemoglobin molecule upon deoxygenation, which leads to end-organ damage manifest as avascular necrosis of bone, stroke and diffuse osteopenia. Vitamin D deficiency is relatively common in African Americans with an estimated prevalence of 21% in the elderly. The explanations for vitamin D insufficiency are likely multifactor - poor nutritional intake, lack of sun exposure and increased skin pigmentation.
