This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. Researchers have found that the death rate in selected high-risk cardiac patients can be sharply reduced with implantable cardioveter defibrillators (ICDs). The current use of ICDs is the outgrowth of basic biomedical research. The NIH-sponsored TOVA (Triggers of Ventricular Arrhythmias) project is a multi-center study for investigating the trigger causes of life-threatening arrhythmias and for evaluating the effectiveness of existing ICDs. It is aimed at strengthening basic biomedical and clinical research. 2000 patients with ICDs are expected to be entered in the study. Electrogram (EGM) and related data interrogated from ICDs have been stored on floppy disks, in proprietary data format that vary widely with the device manufacturer. Currently clinicians and researchers must use different programming devices to plot the data on paper in order to view and analyze them. For this project we are developing customized software for creating an automated, web-based, and searchable ICD EGM and related data archiving system for the TOVA study. This data archiving system allows investigators to remotely upload ICD interrogation data via the Internet. Data from various ICD models of various manufacturers are automatically converted into a standard public format and are inserted into a database system. Researchers can view and download all the EGM waveforms, annotations, and device setting parameters via the web. Investigators can also make notes on any EGM episodes and can search for specific patients or EGM episodes with specified criteria. This new database system is a critical contribution to the TOVA project, greatly accelerating what has been an unsatisfactory method of collecting, organizing and analyzing data. The system also has the potential to accelerate other important research projects that are based on the investigation of the EGM features associated with life-threatening ventricular arrhythmias.
