项目摘要

This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. The principal aim of this study will be to examine alterations in fat cell-derived adipocytokine expression in subjects before and after weight loss. We hypothesize that adipocyte production of metabolic mediators will decrease following weight loss. Subjects will be recruited as outpatients in the Nutrition and Weight Management Center at Boston Medical Center. Eligibility includes those 18 to 80 years old, with a Body Mass Index (BMI) greater than or equal to 25 kg/m^2. Weight loss treatment options will be those offered by the Nutrition and Weight Management Center and can include low fat diets with or without, anti-obesity medications, liquid diets, protein-sparing modified fast, and gastric bypass surgery. Enrollees will have 4 biopsy visits at baseline, and months 3, 6 and 12 during the 1 year observation period. Seven days from each biopsy visit, subjects will be asked to return for a suture removal visit so that their biopsy wounds can be assessed and sutures removed. There will be 4 pairs of biopsy and suture removal visits (8 separate visits total) occurring over the study period. The following procedures will occur at each biopsy visit: laboratory values for CBC, homocysteine, basic metabolic panel, LFT, TSH, C-reactive protein, fasting glucose, insulin, lipids (total cholesterol, HDL, LDL, triacylglycerol), HbA1c, hydroxybutyrate, free fatty acids, leptin, adiponectin, TNF-a and PAI-1; subcutaneous fat biopsy, body composition by DEXA; and urine pregnancy test (for all women). The following will be collected: weight, height, waist circumference, BMI, and blood pressure.
