This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. PTH treatment increases bone formation and spine, hip and total body BMD, but reduces cortical BMD and causes transient small increases in blood calcium. To improve treatment, and insight into the ways PTH increases bone formation, this study compare PTH, PTH+alendronate, and alendronate treatment of osteoporosis in men and postmenopausal women for 30 months. To assess the effects of stopping PTH on bone turnover and BMD, in both the presence and absence of alendronate, no participants take PTH during months 30-42, but those who took alendronate continue it. During months 42-54 they also continue alendronate, and all patients take PTH. Months 42-54 test whether PTH's positive effects on bone formation, which wane after 1-2 years of therapy, resume in full force after a 12 month holiday off PTH.
