This subproject is one of many research subprojects utilizing the
resources provided by a Center grant funded by NIH/NCRR. The subproject and
investigator (PI) may have received primary funding from another NIH source,
and thus could be represented in other CRISP entries. The institution listed is
for the Center, which is not necessarily the institution for the investigator.
There is limited information regarding the presence of cognitive dysfunction in adolescents with type 2 diabetes. There is evidence provided to support the presence of cognitive dysfunction in adults with both type 1 and 2 diabetes. This group has probably had disease for considerably longer than the adolescents. There is some evidence of cognitive dysfunction in children with type 1 diabetes but not those with type 2. In this study, adolescent patients seen in the pediatric endocrine clinic for evaluation and treatment of their obesity will be assessed for 1) the presence of type 2 diabetes, 2) insulin resistance (IR) alone with normal glucose tolerance, and 3) the absence of IR or diabetes. They will then be enrolled into a study that will evaluate potential cognitive defects in those with diabetes using a battery of neuropsychological tests. Finally, they will receive a brain MRI (DT-MRI), looking for differences in myelination and size, in specific regions outlined. The main hypothesis is that there will be lower neuropsychologic scores in adolescents with diabetes. They will also explore correlations between IR, neuropsychiatric test results and MRI variables. Fifty patients are to be enrolled. This pilot study would provide initial data for a subsequent larger study.
