颈椎前路减压融合术(ACDF)是治疗颈椎病的经典手术,但仍存在不能早期融合和早期康复、卫生经济负担重等不足。申请人及团队国内外首创性提出“颈椎钩椎关节融合”理论并获得“颈椎钩椎关节融合器”发明专利授权。申请人前期进行相关应用解剖、生物力学、山羊体内试验等研究发现钩椎关节融合可以实现早期融合、提高融合效率。申请人设想钩椎关节融合实现早期融合可能与融合间距、血管解剖、应力、成骨与促血管机制等有关,并且有望成为临床诊疗新技术促进ACDF术后早期康复、降低卫生经济成本。本研究拟通过融合间距、血管解剖、应力研究、离体骨块培养等手段探究钩椎关节融合机制,优化钩椎关节融合器设计后进行应用探索研究,完善钩椎关节融合理论指导临床诊疗,建立一套新的治疗颈椎病的中国人原创的钩椎关节融合的ACDF手术方案和诊疗体系,进而促进早期融合、提高融合效率及降低卫生经济成本,实现原创成果转化、产品上市和技术应用推广。
英文摘要
Anterior cervical decompression fusion (ACDF) is a classic procedure for the treatment of cervical degenerative diseases. However, the ACDF is still not perfect,how to improve the early fusion rate, accelerate the early recovery of patients, promote the early return of patients to normal production and life, and reduce the burden of social health economics is a vital problem to be solved at this stage of ACDF surgery. Research team proposed the theory of the cervical uncoertebral joint fusion for the first time all over the world, and obtained the national invention patent authorization of the "cervical uncoertebral joint fusion cage"( ZL201510541160.1). A series of preliminary researches such as applied anatomy measurement, biomechanical test, and animal model studies have shown that uncoertebral joint fusion is expected to achieve early fusion and solve the above-mentioned shortage of ACDF surgery. The early fusion of uncoertebral joint fusion may be related to the fusion interval, vascular anatomy, stress, osteogenesis and pro-vascularization mechanisms. This study intends to perform fusion space model research, vascular anatomy research, finite element analysis, in-vivo stress detection, in vitro bone mass culture under stress, osteogenesis and pro-vascularization mechanism in-vivo and in vitro bone mass culture detection, and other means to explore the mechanism of uncoertebral joint fusion; and to perform the design improvement of uncoertebral joint fusion cage, goat animal model study and clinical trial study to explore the value of application and promotion. And finally try to improve the theory of cervical uncoertebral joint fusion, and explore a new set of Chinese original uncoertebral joint fusion’ ACDF for the treatment of cervical degenerative diseases to promote early fusion, improves fusion efficiency and reduces health economic costs, realizes the transformation of original results, product listing and technology application promotion, and ultimately serves patients and society.
