脊柱椎间盘退变是引起下腰痛及颈肩痛的最为常见的病因之一,是目前影响日常健康生活的重要因素。其治疗方式目前仍主要以手术为主,尚缺乏有效的非介入性防治手段,而这与疾病发生机制不明密切相关。近年来研究表明,细胞外囊泡能够通过影响组织微环境参与疾病的发生发展,但其在椎间盘退变中的效应尚不清楚。基于前期研究成果,本项目拟针对退变炎症环境下髓核分泌的细胞外微囊泡进行系统研究,通过比较炎性刺激微囊泡对髓核基质退变的影响明确其功能;利用高通量分析比较炎性刺激下囊泡蛋白及RNA成分的差异,并找出关键分子;通过分子实验明确其影响髓核退变的机制;最终利用构建的退变模型小鼠研究炎性刺激微囊泡以及关键分子对退变的在体影响,并尝试构建以微囊泡为载体的靶向制剂,探究其在体退变修复及预防可能性。通过该项目的实施,力求为进一步明确椎间盘退变的机制提供理论依据,同时为椎间盘退变相关疾病的诊治与预防提供新的思路。

Spinal disc degeneration is one of the most common causes of lower back pain and neck pain, and is an important factor affecting daily health. The treatment method is still mainly surgery, there is still a lack of effective non-interventional prevention and treatment means, and this is closely related to the mechanism of disease occurrence is not clear. In recent years, studies have shown that extracellular vesicles can be involved in the development of disease by affecting tissue microenvironment, but their effects in disc degeneration between vertebral stoics are not clear. Based on the prior research results, this project intends to carry out a systematic study of extracellular micro-vesicles secreted by nucleus pulposus cells in inflammatory environment. To clarify its function by comparing the effects of inflammatory stimulation micro-vesicles on nucleus pulposus matrix degeneration, to analyze the differences in the composition of the micro-vesicle protein and RNA by using high throughput between inflammatory and non-inflammatory environment, and to identify key molecules. The mechanism of its effect on nucleus pulposus degeneration will be clarified by molecular experiments, and the mechanism of nucleus pulposus degeneration was finally studied by using the constructed degenerative model mice to study the in vivo effects of inflammatory stimulation micro-vesicles and key molecules on degeneration, and to try to construct a targeted micro-vesicle carrier therapy method to explore the possibility of repair and prevention of degeneration in vivo. Through the implementation of this project, it is aimed at providing theoretical basis for further clarifying the mechanism of disc degeneration, and providing new ideas for the diagnosis and treatment and prevention of intervertebral disc degeneration-related diseases.
腰痛及颈肩痛作为现代社会最为常见的疾患之一,严重影响着人们的生活质量。椎间盘退变及其继发病变是最重要的影响因素之一。椎间盘发生退变时早期变化实质为髓核细胞外基质的稳态破坏,炎症反应是最重要的发病机制之一。为进一步明确退变过程中影响髓核细胞基质代谢以及微环境稳态平衡的因素,我们选择了近年来广受关注的微环境调节物质:细胞外微囊泡(EV)。EV携带一系列信号传导蛋白、信使RNA以及 microRNA 等分子并发挥广泛的生物学效应,参与并影响众多生理病理过程。在椎间盘领域,绝大多数研究主要针对干细胞来源EV对于椎间盘退变的修复作用,但有关椎间盘自身分泌的EV 对退变影响的研究则较少,显示出EV对椎间盘退变作用和机制研究的缺乏。研究不同刺激下髓核分泌EV对髓核ECM代谢的作用研究将有利于破解EV对髓核退变作用的关键机制,为未来保守治疗研究的开展提供重要的理论依据,同时也为椎间盘退变的非介入性治疗的探索提供重要线索。本项目中,项目组首次多方面比较了体外不同刺激下髓核细胞分泌的细胞外微囊泡的表达差异。通过生信分析,发现炎性因子的刺激与正常退变髓核细胞的表达谱最为相似。接着验证了不同刺激下髓核细胞分泌的外泌体对正常髓核细胞的作用,并进一步发现了退变髓核外泌体可以通过调节GATA4的表达作用于NF-κB通路,进而促进椎间盘退变。水凝胶是外泌体的良好载体,可以提升外泌体的作用效率和时间,有广阔的应用前景。项目发表基金标注SCI期刊论著15篇,其中2篇为影响因子10分以上的高影响力期刊论著。发表2篇中文期刊论文,培养硕士、博士研究生4名。项目实施所明确的不同退变环境刺激下髓核细胞分泌EV成分差异对于髓核ECM代谢的影响以及其具体机制,为今后相关靶向药物的开发提供重要理论支持。
