Epstein-Barr virus infection in Chinese salivary cancers

为了解中国地区涎腺淋巴上皮瘤中EB病毒的感染情况，我们在台北、台中、成都、乌鲁木齐、济南等地进行了前期工作，收集了159例涎腺淋巴上皮癌标本，采用免疫组织化学、EBER-1原位杂交和EBD DNA BamHI W区聚合酶链式反应技术进行了EB病毒的检测。3，西安；2，乌鲁木齐；0，济南；6，台北；3，台中。沿海地区发病率较高。成都的这一数字似乎与过去来自西南沿海的频繁移民有关。淋巴上皮瘤包括74名男性和78名女性患者(9例不明)。除1例维吾尔族和1例锡伯族外，其余均为汉族(3例不详)，年龄9~77岁，平均40.5岁，年龄<40岁者占35%。它的发生在最年轻的一代是有特点的。超过一半的病例位于腮腺(69%)，14%位于小涎腺。涎腺淋巴上皮瘤的癌细胞EMA、CA15-3和UEA-I结合呈强阳性，提示其导管上皮性质。胞浆中偶见LMP-1免疫阳性。所有受检者的细胞核内均有EBER-1的强信号。间质中以T细胞为主。大多数病例的扩增产物也呈阳性。但在良性淋巴上皮瘤中未发现EBV信号，提示EBV和地理因素在涎腺淋巴上皮瘤的发病机制中起重要作用。

In order to determine prevalence of Epstein-Barr virus (EBV) infection in salivary lymphoepitheliomas in China, we visited Taipei, Taichung, Chengdu, Urumqi, and Jinan, inaddition to the eight points where we had made expeditons previously and collected total 159 cases of this type of carcinomas and examined the tissue samples by immunohistochemistry, in-situ hybridization (ISH) for EBER-1 and polymerase chain reaction (PCR) technique for EBD DNA BamHI W region for the presence of EBV.The 159 cases of salivary lymphoepitheliomas were composed of 42 cases from Shanghai, 40, Chengdu ; 42, Guangzhou ; 9, Beijing ; 4, Wuhan ; 4, Changchun ; 3, Kunming ; 3, Xi'an ; 2, Urumqi ; 0, Jinan ; 6, Taipei ; and 3, Taichung. The high incidences were noted along the coast area. The figure from Chengdu seems to be related to frequent immigration from the South-West coast in the past. The lymphoepithelioma series consisted of 74 male and 78 female patients (9 unknown). Except for one Uygur and one Xibe, the patients were of Han race (3 unknown), ranging in age from 9 to 77 years (mean, 40.5 years ; 35% of patients were younger than 40 years). Its occurrence in the youngest generation was characteristic. More than half of the cases were located in parotid glands (69%), and 14% in minor salivary glands. Carcinoma cells of salivary lymphoepitheliomas were strongly immunopositive for EMA, CA15-3, and UEA-I binding, indicating their duct epithelial natures. Their cytoplasm was occasionaly immunopositive for LMP-1. The nuclei showed intense signals for EBER-1 by ISH in all of the cases examined. T-cells were predominant in the stroma. Positive PCR products were also shown in most of the cases. However, no EBV signals were obtained in the benign lymphoepithelial lesions.The results suggest that EBV and geographic factors play an important role in pathogenesis of the Chinese salivary lymphoepitheliomas.
