This subproject is one of many research subprojects utilizing the
resources provided by a Center grant funded by NIH/NCRR. The subproject and
investigator (PI) may have received primary funding from another NIH source,
and thus could be represented in other CRISP entries. The institution listed is
for the Center, which is not necessarily the institution for the investigator.
Two defining features of the metabolic syndrome (MetS) are high triglycerides (TG) and low HDL (dyslipidemia of MetS). At weight stability, there is no consensus on the dietary fat and carbohydrate proportions best suited for managing the dyslipidemia and the impaired vascular reactivity of MetS.
In contrast to extreme high and low-fat diets, diets moderate in fat and carbohydrate intake may be more beneficial in MetS based on recent studies. Overweight and insulin resistant persons appear to be less responsive to saturated fat restriction than normal weight persons. Consistent with these findings are lower rates of coronary disease in persons ingesting higher fat diets in observational studies.
This study will compare lipoprotein and vascular effects of two diets in a randomized, 4-week, crossover, feeding study design, using food prepared in the metabolic kitchen of the UW GCRC. Diets will contain 40% fat, 45% carbohydrate or 20% fat, 65% carbohydrate with equivalent protein (15%) and saturated fat (8%). Weight will remain stable. Seventy-two men and women will be randomized to yield a minimum of 60 finished subjects at a 20% drop out rate. Weight and other vital signs are measured twice weekly when food is picked-up from the GCRC. Lipoprotein lipids are measured at 0, 3 and 4 weeks. Apoproteins B and A-I, LDL buoyancy, glucose, insulin, adiponectin, inflammatory cytokines and flow-mediated brachial artery reactivity are measured at 0 and 4 weeks. A 4-month extension will test the durability of the effect of the second diet assignment under dietician counseling.
