Eyedrops have been proposed for the noninvasive systemic delivery of insulin. 1-7 Absorption enhancers such as surfactants are usually included in the dosing solutions to facilitate the uptake of insulin into the bloodstream. In 1989, Lee and co-workers carried out a thorough study in determining the extent and pathway of the systemic absorption of insulin after topical instillation of insulin eyedrops to the albino rabbit. 1 They found that the majority of insulin is absorbed systemically from the nasal mucosa and to lesser extent from the conjunctival mucosa. Although the ocular delivery of insulin from eyedrops is possible, it is subject to poor and variable bioavailability and a short duration of activity. The problems associated with ophthalmic solution delivery can be overcome through the use of an appropriate ocular device. The objective of this study is to develop a controlled-release device for the systemic delivery of insulin via the ocular route in rabbits. The proposed controlled-release device utilizes the absorbable gelatin sponge (Gelfoam, Pharmacia & Upjohn Company) as the insulin carrier. When Gelfoam absorbs water (or tears), it becomes soft and pliable. This is an important factor in making it comfortable in the eye. It has been shown by Nadkarni and Yalkowsky to be an excellent drug carrier for delivering pilocarpine locally to the eye. 8 It is common practice in the evaluation of insulin delivery to assess the efficacy by measuring the blood glucose levels. 1, 5, 9-12 In this study, blood glucose levels are used to assess the relative efficacy of ocular insulin delivery systems. Gelfoam sponge, size 100, was obtained from Pharmacia & Upjohn (Kalamazoo, MI). Sodium bovine insulin (26.0 IU/mg) was purchased from Calbiochem Corporation (La Jolla, CA). A ONE TOUCH BASIC blood glucose monitoring system was obtained from Lifescan Company (Mountain View, CA) as a gift and was used to analyze the blood glucose concentration. All other chemicals were used as received from commercial suppliers.Insulin eyedrop formulations were prepared by dissolving either 0.5 or 1.0 mg of sodium insulin along with 20 μg of Brij-78 which acts as an absorption enhancer in 30 μL of freshly distilled filtered water. Eyedrop solutions were prepared fresh immediately before each experiment. The devices were prepared by sorbing either of the above eyedrop solutions into the preweighed Gelfoam sponge (approximately 6 mm diameter and 2 mm height) and then drying under vacuum for 72 h. The dried devices were weighed to confirm their insulin content.
