An important confounder in the diagnosis of FH is the certain contribution of the cholesterol content of Lp (a) in individuals with elevated Lp (a) because Lp (a)-C is incorporated into clinical LDL-C measurements. Lp (a)-cholesterol [Lp (a)-C] is the amount of cholesterol trafficked by Lp (a) particles and is expressed as mg/dL. Lipoprotein (a)[Lp (a)], discovered in 1963, is composed of apolipoprotein (a)[Apo (a)] bound covalently to apolipoprotein B (ApoB) of a low-density lipoprotein (LDL)-like particle. 1, 2 Over the past decade, epidemiological, Mendelian randomization and genome-wide association studies have established Lp (a) as an independent and likely causal risk factor for atherosclerotic cardiovascular disease (ASCVD). 3-6 However, the clinical importance of Lp (a) was not defined because of a lack of both reliable medication for Lp (a)-lowering therapy and definite evidence that lowering Lp (a) levels can reduce ASCVD risk for a long time. 17 Yeang C, Willeit P, Tsimikas S.
