新型コロナウイルス感染症（COVID-19）の流行を契機に、急速に発展しているオンライン診療であるが、歯科領域は依然として大規模には普及していない可能性がある。そこで本年度は医科と比較して普及が進まない事由を探るため、①診療科別受診動向分析、②オンライン診療アプリケーション（アプリ）の比較、③ヒアリング調査、を実施した。なお、本研究では基本的に「Dentist to P」という1対1で歯科医師が患者を診療する形態を想定している。まず、オンライン診療実施の有無にかかわらず、診療所の診療科別の外来受診動向を明らかにした。概算医療費データベースの2009年1月から2021年12月（13年間）のレセプト件数を利用して、Joinpoint回帰モデルにより長期的な動向を観察するとともに、COVID-19流行下の変化を分析した。その結果、COVID-19流行直後である2020年4-5月は歯科の外来受診の減少が顕著であったが、2019-21年の年変化率は他の診療科に比べて減少の幅が少なく、13年間の平均年変化率は統計学的に有意に増加していた。次に、オンライン診療アプリに関する比較調査を行った。歯科領域で利用が可能なオンライン診療アプリ・システムを３つ選択し、ウェブ上の掲載情報や資料請求により入手した情報により比較を行った。調査項目は、導入までの日数、初期導入費用、利用料金、処方箋連携、ビデオ通話利用システム、セキュリティ、導入後のサポート等とした。その結果、アプリ間で大きく異なったのは、導入までの日数と料金体系であった。導入までの日数は当日から約２週間とアプリによって差があり、利用環境や料金体系によって異なる可能性が示唆された。ヒアリング調査では、昨年度から引き続き事例収集を行い、摂食嚥下リハビリテーションや義歯装着後のフードテストにおける活用の可能性、その課題が明らかになった。

The number of estimated medical expenses from January 2009 to December 2021 (13 years) was analyzed in terms of utilization, long-term trends, and changes in COVID-19 prevalence. As a result, the decrease in external visits to dentistry from April to May 2020 has been significant since the COVID-19 epidemic began, and the annual rate of decrease in dentistry from 2019 to 21 has been less than that of other medical departments. The average annual rate of decrease over the 13-year period has been statistically significant. A comparative study was conducted on the secondary and secondary diagnosis.
