本研究では、臨床実習学生が診療参加型臨床実習を行うための基盤となる汎用的能力について何が不十分であるか、さらにその状況を踏まえて医学教育をどのように進めるべきかということを学術的「問い」として設定している。令和4年度は、前年度に引き続き、必要とされる汎用的能力を明らかにするために、学内必修の診療参加型臨床実習の履修を修了した5年次を対象として、担当患者の診察、診療録記載、指導医へのコンサルテーション、診療チームへの参加、カンファレンスでの症例提示等の診療参加において、無記名の自己評価アンケート調査を実施した。さらに、すべての診療参加型臨床実習の履修を修了した6年次を対象として、卒業時コンピテンス・コンピテンシーの達成レベルにつき自己評価を実施した。診療参加型臨床実習における学生の自己評価（1～5点）では、病歴聴取3.13点、身体診察3.08点、診療録記載2.95点、症例提示3.19点、診療手技3.16点、診療チームへの参加3.31点であり、前年度と同様に診療録記載の評価が最も低かった。また、自由記載では、記載例の76.2%が専門的知識・技能に関するもので、汎用的能力に関連するものは12.5%しかなかったが、診療力記載、コミュニケーション、プレゼンテーション、ディスカッションがあげられていた。卒業時コンピテンス・コンピテンシーの達成レベルについては、前年度と同様に、医療経済・政策・保険診療の理解と保健・医療・福祉・介護および行政等との連携協力、適切な統計手法の選択と解析、国際的コミュニケーションや英語を用いた情報の入手・理解・発信が他の項目よりも低かった。これらの結果から、現在の臨床実習学生においては、コミュニケーション力・協働力、文章力、プレゼンテーション力、ディスカッション力、国際力等の向上が必要であることが示唆された。

In this study, clinical practice students participated in the diagnosis and treatment of clinical practice, and their basic skills were widely used. In 2004 and 2005, the Department of Health conducted a comprehensive review of the ability to participate in clinical practice, which is required to participate in the study, and revised the clinical practice for 5 years. The Department of Health conducted patient examination, recorded medical records, guided medical treatment, participated in medical treatment, and provided case information. The investigation was conducted anonymously. In addition, the practice of participating in clinical practice has been revised for 6 years, and the practice of self-evaluation has been carried out at the end of the study. Clinical practice of diagnosis and treatment participation type: student's self-evaluation (1 - 5 points), disease history: 3.13 points, physical examination: 3.08 points, diagnosis record: 2.95 points, symptom suggestion: 3.19 points, diagnosis manual: 3.16 points, diagnosis and treatment participation: 3.31 points, previous year: same diagnosis record: lowest evaluation. 76.2% of the recorded cases were related to knowledge, skills and general abilities of the subject, and 12.5% were related to medical skills, skills and abilities. Completion of the study, previous year, and the same, medical care, policy, health care, well-being, care, administration, etc., and coordination, appropriate statistical methods, selection and analysis, international study, and English use of information, understanding, and development of other projects. The results of this study show that there is an upward trend in the development of clinical practice students, such as the development of Chinese medicine, and the development of Chinese medicine.
