2022年度は2022年1月から臨床実習を開始する4年次に対して新しい診療現場指導・評価方法を導入し検証を行う予定であったが、COVID-19感染症流行によって、2022年1～12月の4～5年次必修のクリニカル・クラークシップは45週中11週が対面実習中止あるいは患者との接触禁止となり、それ以外の期間においても、患者との接触は1日15分以内に制限された。このように学生による診療参加がほとんど実施できていない状況において、必修実習を修了した5年次を対象にアンケート調査を実施し、臨床実習実績および自己評価、指導についての学生の評価、学生の満足度、自由記載意見等のデータをコロナ禍前の2019年度と比較した。この結果、臨床実習実績は、担当患者数-15.6%、病歴聴取患者数-45.6%、身体診察患者数-50.2%、カルテ記載患者数-51.2%、症例提示患者数-60.6%、病状説明立会患者数等-39.0%と大幅に減少し、学生による評価（1～5点）では、病歴聴取、身体診察、カルテ記載、症例提示、医療手技、診療参加、指導体制・状況のいずれも有意に低下していた。しかしながら、総合的満足度（100点満点）は85.6点で、2021年度よりもさらに上昇し、2018-2019年度平均の77.1点よりも有意に高かった。自由記載では、対面実習中止による診療参加の減少を残念に思う意見が多数見られる一方で、レクチャー中心となった臨床実習のために知識を修得できたことを肯定的に捉える意見が多くみられ、これが総合的満足度の上昇に繋がったと考えられた。これらの結果は、学生は、診療参加による技能・態度領域の修得よりも、知識領域の修得に関心が高いことを示すものであり、新たな診療現場指導・評価法導入による教育効果の検証には、学生の自己評価や満足度ではなく、技能・態度領域の到達度指標が必要であることを示唆するものである。
