项目摘要

本研究では、治療と職業生活の両立支援における、疾患毎の復職課題の特性について患者コホート調査を行い、この成果から得たエビデンスを基に、早期復職・安定した就労継続を目指した両立支援プログラムの提案を行う。最終的に、本研究は休業期間を短縮し、本人と職場ひいては日本の経済活動に貢献する。本研究の目的は、治療と仕事の両立支援における疾患毎の課題の発生や困難性の偏在を明らかにすることである。最終目標として、この知見に基づいた、両立支援コーディネーターによる「復職課題」への早期介入アプローチとして効率的かつ実行可能な支援策を考案する。2021年度は、後向きコホート研究の倫理申請および症例データベースの整備した。2022年度は、職課題の評価指標の問診票を整備及び現場でデータ収集を開始した。並行して、作成したデータベースで解析を開始した。精神的苦痛や職場配慮と、当事者の治療と就労の両立に対する満足度(以下、両立満足度)との関連性について評価した。産業医科大学病院において2019年1月-2021年3月治療時に両立支援を希望し初回面談を実施した837例に対し、追跡調査を行った。2021年11月-2022年3月の期間に再診した症例のうち、治療と就労の両立に関する問診にて就労継続中であった182例を解析対象とした。説明変数は精神的苦痛と職場配慮とし、アウトカムは両立満足度とし、ロジスティック回帰モデルを用いてオッズ比を算出した。解析対象182例において、平均年齢50.6歳、男性は43.5％、精神的苦痛K6は平均2.67/23点であった。精神的苦痛が高いと治療と就労の両立状況に満足するオッズ比が有意に低い傾向があった。また、同僚からの支援が高いと両立状況に満足するオッズ比が有意に高い傾向があった。業務調整なし群と比較し、業務調整あり群では両立状況に満足するオッズ比は高い傾向があった。上司からの支援と両立満足度については関連性を認めなかった。
