项目摘要

本研究の目的は、心不全患者のセルフケア能力向上のために、患者の身体経験に基づいたセルフケア支援方法を開発し、その評価を行うことである。具体的には、心不全患者のセルフケア能力向上のために、１）心不全急性増悪による緊急入院から退院後の生活（およそ半年間）における身体経験を調査する。２）１）の分析結果とこれまでに作成した「慢性心不全患者の症状体験に働きかけ、セルフモニタリングを強化する看護支援」および文献を基に、『心不全患者の身体経験に基づいたセルフケア支援方法』を作成し、３）2施設の循環器病棟と外来で試行して、セルフモニタリングおよびセルフケア能力、退院後2年間の再入院率を評価する予定である。令和3年4月1日～令和3年3月1日迄産前産後・育児休業のため、1年間研究活動を休止した。令和4年4月1日より、心不全急性増悪による緊急入院から退院後の生活における身体経験の調査を行った。分析の結果、心不全急性増悪による急激な身体の変化とともに、心不全患者の身体性、関心、時間性が変化する様相が明らかになってきた。また、心不全患者の身体経験には、多職種のセルフケア支援が潜在的、顕在的に様々な影響を与えていると考えられた。そのため、『心不全患者の身体経験に基づいたセルフケア支援方法』を検討するためには、看護職のみでなく、多職種協働による支援が必要と考えられた。そこで、「心不全患者の身体経験と多職種セルフケア支援の実態」についての追加調査を行うこととなった。現在、調査結果とこれまでに作成した「慢性心不全患者の症状体験に働きかけ、セルフモニタリングを強化する看護支援」および文献をもとに、『心不全患者の身体経験に基づいたセルフケア支援方法』を作成中である。
