本研究の目的は、訪問看護師の終末期がん症状マネジメント教育プログラムを開発し、その有効性を検討することである。2022年度は①がん症状マネジメントに対する訪問看護師の困難感②訪問看護師のがん疼痛マネジメントの現状に関する文献調査を行った。①は、「終末期」「がん」「訪問看護」の検索語を特定し、医中誌Web版とPubMedを用い、過去10年の原著論文を検索した。その結果、279件中16件が該当し、がん疼痛を緩和するためのケアや看護師の判断力が求められること、他職種との連携に困難感を抱くことが分かった。また、2022年の日本におけるがん看護研究の優先性に関する調査結果から、患者にとって最も苦痛、かつ、看護師にとってマネジメント困難な症状として、がん疼痛は依然として上位にあげられており、がん疼痛マネジメントに焦点化して研究を進める方針とした。②は、医中誌Web版とPubMedを用いて文献検討を行った。看護師・がん性疼痛・態度or知識or自信or実践・訪問看護に関連する検索語を特定し、検索を繰り返した。40件が抽出され、態度・知識・自信・実践のいずれかについて量的データで報告があった5件を分析対象とした。その結果、2008年2015年と全国調査が行われ、訪問看護師の症状緩和に関する困難感は減少傾向であったが、疼痛に関する知識・緩和ケアの実践頻度に変化はなかった。また、2015年の全国調査の後、近年の変化について詳細は明らかとならなかった。上記2つの文献調査の結果、がん疼痛マネジメント教育プログラムの開発にむけて、訪問看護師のがん疼痛マネジメントの実践の現状と、具体的にどのような場面で訪問看護師が困難を感じ、関連するバリアの把握が、効果的なプログラム内容を検討するために必要と考えられた。よって、まずは実態調査を行うこととし、倫理審査に向け計画書作成を開始した。

The purpose of this study was to investigate the development of end-stage symptom management and education for visiting nurses. A literature survey on the status quo of pain management for visiting nurses was conducted in 2022. 1. Search for "terminal","visiting care","medical journal Web edition","PubMed","original papers" in the past 10 years. The results showed that 16 of the 279 cases were related to the pain relief and the judgment of the caregiver. The results of the literature survey, the development of the pain management system, the current situation of the practice of the pain management system, the specific situation, the difficulty of the interview nurse, the relationship between the grasp of the pain management system and the results of the education system are described in the paper. The preparation of the project plan for ethical review shall be started at the beginning of the investigation.
