本研究の目的は、都市部の診療所外来において、看護師がどのような生活に関わる課題に気づき、支援に向けた取り組みをされているのか明らかにすることである。研究対象は、都市部の診療所に従事している、診療所での経験が通算5年以上の看護師である。本研究では生活に関わる課題については、痴呆の進行、老々介護、介護困難、育児困難、生活困窮、引きこもり、虐待、ＤＶ、性暴力被害など様々な生活に関わる解決すべき問題であると定義している。面接の際には、生活に関わる課題について、「気になる」という表現を使用している。面接内容は、診療所外来での通算の経験年数と、診療所が標榜している診療科を尋ね、どのような患者さんやご家族に対して、気になると感じるのか、対応が必要であると感じた場合の対応内容、現在困難に感じていることを尋ねる。今年度末までに、10名の看護師に協力が得られ、面接調査を実施している。面接時間は、30分から45分程度である。現在、5名の看護師より協力の意思が確認できており、今後、面接予定である。分析はコード化のみではあるが、面接内容より、都市部においても、地域の特性、診療所が持つ機能により、生活課題への対応がかなり異なることが見えてきた。来院する患者の特性が、診療所の地域の居住者か、就業者かにより、地域との連携の仕方が容易となる場合と、困難になる場合があることも明らかとなってきた。今後は、分析をさらに進め、今年度末までに、論文としてまとめたい。
