项目摘要

訪問看護師に必要な判断する力について、帰納的アプローチにより訪問看護師の「判断」が導かれる思考プロセスを概念化した。訪問看護師の判断に至るためのプロセスは、①手がかりを感じ取る力（関心をよせる、気づき、疑問を持つ）、②見えないことを推論する力（病状と生活を想像する、情報を分析し解釈する、複数の仮説を立てる）、③考えを言葉にする力（対話を積み重ねる、身体の声を翻訳する、予測されることを伝える）、④余計なことをしすぎない力（自他の価値観を区別する、力を信じて待つ、背中を押す、撤退する）の４つの思考サイクルを基に、その①から④を統合して導く⑤最善解を導く意思決定の共有によって構成された（清水、2022）。本研究における訪問看護師の判断力とは、医学的根拠に基づき推論する予測的判断と対話に基づく価値判断をすり合わせ、療養者の思いを多職種連携の中心に置き、その時の状況、その場に合った療養者と家族にとっての最善解を導くことであった。本研究で概念化した訪問看護師の判断が導かれる思考サイクルを基盤としたケースメソッド教材を作成した。この教材を用いて新任期訪問看護師を対象に研修プログラムを実施した。プログラム内容は、①訪問看護師に必要な「判断力」、②判断力を鍛える方法・臨床推論、③事例を用いた判断力トレーニングと振り返り、④実践報告と学びの共有を行った。教材開発にむけた研修プログラムの成果と課題を分析している。

This study conceptualizes the judgment of visiting nurses and creates teaching materials for them. This teaching material is used to interview nurses for a new term. The content of the interview is: ① judgment necessary for visiting the nurse; ② judgment training; method; clinical inference; ③ case; judgment in use; ④ practice report; learning; and common practice. The results and problems of teaching material development are analyzed.
