世界中で腹腔鏡下スリーブ状胃切除術（LSG）をはじめとする肥満外科・糖尿病外科治療は、現代の肥満および2型糖尿病の治療に恩恵をもたらすと期待されている。しかし術後、減量効果や2型糖尿病改善効果が劣る症例も認め、肥満や2型糖尿病はいまだに治療が困難な疾患である。近年腸内細菌叢と肥満や2型糖尿病の関連が注目されており、特にAkkermansia muciniphila菌は耐糖能を向上させると報告されている。このことから腸内細菌叢が術後の効果に関与している可能性がある。本研究の目的は、腸内細菌叢（特にAM菌）が肥満・糖尿病外科手術後減量効果・糖尿病改善効果に関与しているのではないかという仮説を実証することである。具体的には、腹腔鏡下スリーブ状胃切除（LSG）術前後で腸内細菌叢を調べ、術後の減量および糖尿病改善効果に影響する因子を多施設共同で探求・同定・解析する。対象は、7つの肥満症外科手術認定施設（東北大学、大分大学、東邦大学医療センター佐倉病院、関西医科大学、千葉大学、大阪大学、当院）でLSGを行う肥満を伴った2型糖尿病患者である。手術適応は、年齢が18歳から65歳までの原発性肥満であり、BMIが35kg/m2以上で２型糖尿病を認めるものとする。症例数は、7施設で計60例を目標とする。令和4（2022）年度は、本研究の前段階の研究として行った減量効果不良因子の研究において、術後3か月の減量の状態が、術後1年の減量の状態に強く影響することが判明した。この知見に関する発表を行い、本研究の各種項目の測定時期として、術後3か月が適切であることの根拠が得られた。また、新型コロナ感染症蔓延の状況等のためにこれまで入手ができなかった試薬を購入することができた。しかし研究代表者が新型コロナ感染症に罹患し、研究の進行が遅れた。今後、検査体制を整え、7つの施設でLSGを行う肥満を伴った2型糖尿病患者を登録し、各種検査とLSGの施行を行っていく予定である。

Laparoscopic gastrectomy (LSG) in the world is expected to be performed in the treatment of type 2 diabetes mellitus. Type 2 diabetes mellitus is difficult to treat. In recent years, the relationship between intestinal bacterial flora, obesity and type 2 diabetes has been noticed. In particular, Akkermansia muciniphila has been reported to have increased glucose tolerance. Intestinal bacterial flora is related to the possibility of postoperative complications. The objective of this study was to demonstrate the relationship between intestinal bacterial flora (especially AM bacteria) fertility, reduction of diabetes after surgery, and improvement of diabetes. The specific factors affecting the regulation of intestinal flora before and after laparoscopic gastrectomy (LSG), postoperative reduction and improvement of diabetes mellitus were investigated and analyzed together. For patients with type 2 diabetes mellitus, LSG is a certified surgical facility for obesity (Tohoku University, Oita University, Toho University Medical Center Sakura Hospital, Kansai Medical University, Chiba University, Osaka University, and local hospitals). Surgery is appropriate for patients aged 18 to 65 years old with a BMI of 35kg/m2 or more who have type 2 diabetes. The number of cases was 7,60 cases were designed. In 2022, the study on adverse factors of reduction in the first stage of this study, the state of reduction in 3 months after surgery and the state of reduction in 1 year after surgery were identified. The results of this study are as follows: (1) The time of measurement of various items in this study is 3 months after operation, and (2) The time of measurement is 3 months after operation. The situation of the spread of new infections, etc. The representative of the research team is the new type of infectious disease, and the research is ongoing. In the future, the inspection system will be improved, the LSG will be implemented in 7 days, and patients with type 2 diabetes will be able to log in. Various inspections and the implementation of the LSG will be carried out in advance.
