项目摘要

2020年度は、スタンフォード大学で作成された心理社会的評価を目的としたThe Stanford Integrated Psychosocial Assessment for Transplantation(以下 SIPAT)尺度を本邦で利用するために、SIPATを日本語版を作成し、内的妥当性と外的妥当性を検証し、中間解析を報告した。2021年度は最終解析を行い、評価者間一致率は（ExpertとNovice）、尺度間一致率（日本語訳SIPTとJ-PACT）が十分に高いことを示した。以上より、日本語訳 SIPATは評価者の移植面接の経験を問わずに一定の心理社会的評価を可能とすることが示唆され、日本語訳SIPATの高い評価者間信頼性、尺度関連妥当性を示した。以上の内容について、国際学会誌であるJ Acad Consult Liaison Psychiatryに投稿し受理された (PMID:3486909)。2022年度は、まず、2021年度中に追加エントリーされた対象を含めたデータクリーニングとデータベース構築を行なった。完成されたデータベースを用いて、1）予後予測妥当性の検証、2）臓器移植前植込み型人工心臓植込み後のメンタルヘルスの調査、3）臓器移植前患者の認知機能の調査を行なった。1）は腎移植後のメンタルヘルスとSIPATの特定項目とに有意差があり一定の予後予測可能性が示された。2）は、植込み型人工心臓植込み後の適応障害発症リスクが増加する可能性が示唆された。3）は、全般的に臓器不全患者の認知機能が低下していることが明らかになった。いずれも国際誌に投稿準備中である。
