周産期医療は、産科・産婦人科医師以外の医師が担当することがなく、他の診療科医師では代替が利かないこと、産科・産婦人科医師は若年世代ほど女性比率が 高いことなど他の診療科とは異なった特徴がある。当該診療科は時間外労働が多く、医師の働き方改革で検討されている労働時間の短縮を適応した場合、医療体制の維持が困難であると予想されている。また、高齢出産に伴うハイリスク分娩の増加による医師の業務負担や出生数減少による影響についても検討が必要である。本研究では、産科・産婦人科の特性を踏まえ、その需給と偏在の現状について分析モデルを作成し客観的にデータを用いて提示すること、将来の労働時間の短縮や女性医師の割合が増加することを踏まえた、必要とする産科・産婦人科医師数について推計を実施することを目的としている。 2020年の研究では、文献検索を行い、医師の偏在について量的データで全国を対象に検討されていた6文献を対象とした。悉皆調査とアンケート調査に大別され、悉皆調査では医師・歯科医師・薬剤師調査のデータを用い、診療科別の医師数の推移、地域格差について検討され、産婦人科では医師数は減少し、地域格差が大きいことが示されていた。文献調査等の結果を踏まえ、産科・産婦人科のある医療機関へのヒアリング項目を作成した。2021年の研究では、医師の供給量、医師の集約度、妊婦の分娩に伴う移動の把握について、使用するデータ項目を抽出し、市区町村単位と二次医療圏単位毎に分析モデルを作成した。目的外申請により官庁統計を入手しデータセットを作成した。都内の産科クリニックに対してヒアリング調査を実施した。2022年の研究では、官庁統計のデータ解析途中であり作成した分析モデルに当てはめて検討中である。都内の産科クリニックに対するヒアリング調査結果のまとめを行い、常勤医師の負担が大きいことが明らかとなった。
