研究は、心不全重症化予防にむけた患者教育を成果（アウトカム評価）の視点から捉えなおし、地域で完結する医療提供体制での心不全患者のための教育連携プログラムを開発するものである。令和4年度は、評価指標となる①心不全患者（ステージB.C）に対して行っている教育支援とその評価内容・方法について　②他施設と連携したツールの先行研究の分析を行った。結果：セルフケア行動であるセルフモニタリングとセルフマネジメントについての教育支援が行われていることが報告されていた。また、評価は、教育支援する前の理解度やセルフケア行動の現状を把握するためや指導後の理解度・実践状況を把握することを目的に実施されていることも明らかとなった。評価方法は、様々な評価尺度が用いられていた。連携は、カンファレンス、クリニカルパス、情報提供書、クリニカルパス、心不全手帳などによって行われていた。カンファレスでは、院内、他施設とのカンファレンスでは患者の情報共有および一貫した支援の調整といった事例が多くなされていた。クリニカルパス、情報提供書では患者の病態・治療の情報共有・管理、治療スケジュールといった方向性の共有ツールとして活用されていた。心不全手帳は、心不全の状態のセルフモニタリング、セルフケア行動につながるための学習支援のツールとして用いられていた。心不全手帳は、学習支援としてのツールおよび自己管理のツールとして活用されおり、患者が心不全手帳を活用できるということは、患者教育の成果であり、アウトカムの評価指標の1つであるといえる。しかし、多施設間で心不全手帳を連携での際にどのように用いているかは調査が必要である。結論：心不全患者に対して行っている教育支援とその評価として、心不全のセルフケア行動の各要素の程度を評価指標とし、教育連携の方法としては、連携ツールの具体的な活用状況を検討する必要性が明確となった。
