项目摘要

これまでの糖尿病性腎症進行予防のための介入研究は，糖尿病性腎症の病期は1期～5期と幅広く，病期を移行した対象への介入研究も多い．そのため，病期を特定した介入研究が少なく，特に糖尿病性腎症初期の患者に特化した腎症進行予防のための介入プログラムのエビデンスはほとんどない．また，わが国において，糖尿病性腎症患者対象の患者教育プログラムについての研究報告も少なく，糖尿病性腎症の進行予防のための介入内容のエビデンスを創出することは喫緊の課題である．本研究では，糖尿病性腎症初期患者を対象とした，腎症進行予防，腎症の疾病認識とセルフマネジメント能力の向上を目指した患者教育プログラムの試案を開発し，フィージビリティスタディを実施することで，プログラムの臨床応用の実行可能性を検討することを目的とする．今年度は，患者教育プログラム（案）の作成を行った．参考としている英国の糖尿病教育プログラムであるDESMOND教育，それを用いたDESMONDスタディやMEMOスタディ，先行しておこなった文献レビューや糖尿病性腎症初期患者の透析予防指導の受け止めに関する質的研究の結果，治療ガイドライン等をもとに，患者教育プログラムの理論的枠組み，構成要素，介入方法，期間，アウトカム指標を検討した．今後は，作成した患者教育プログラム（案）の形成的評価，その後のプログラム（案）の修正，修正した教育プログラム（案）のパイロット調査を経て，教育プログラムを用いた介入のフィージビリティスタディの実施計画を進めていく予定である．

This study aims to develop a trial for early diabetic nephropathy patients, to prevent the disease, to raise awareness of the disease, to improve the ability to develop the disease, and to explore the possibility of clinical use of the disease. This year, the patient education program was launched. DESMOND Education in the UK, including the use of DESMOND and MEMO education, is the first to review the literature and the results of qualitative research on dialysis prevention guidance for patients with early diabetic nephropathy. In the future, the evaluation of the formation of the patient education program, the revision of the program, the revision of the education program, the investigation of the program, the intervention of the education program, and the implementation plan will be determined.
