2022年度は、脾臓摘出術（以下脾摘術）を行った患者背景を明らかにすることを目的として厚生労働省が公開しているNational Database（以下NDB）からデータを収集し、記述統計を行った。2015から2019年の脾臓切除を含む手術の総件数は21931件で、手術の分類名称としては、膵体尾部腫瘍切除術（脾同時切除）、脾摘出術、腹腔鏡下脾摘出術が抽出された。年齢区分では、70から74歳、65から69歳、75から79歳の順に多い結果であった。性差で比較すると、15から50歳までは女性に多い特徴があり、55以降は男性に多い特徴がみられた。NDBの分析結果からは、脾摘に至った原因疾患との関連を明らかにできなかったため、脾摘術の適応となる疾患の特徴を検証した。15から50歳までの女性に脾摘術が多く実施されている要因として、特発性血小板減少性紫斑病との関連が考えられた。また、55歳以降の男性に脾摘術が多く実施されている要因としては、胃がんや肝硬変に伴う門脈圧亢進症の関連が考えられた。今後、特発性血小板減少性紫斑病によって脾摘に至った患者を研究対象者として、患者教育に活用できる学習支援システムの構築を進めていく予定であり、特発性血小板減少性紫斑病の患者会への調査協力を依頼している段階である。患者教育用のデジタル教材の作成をする上で、感染予防行動をアウトカムとし、患者教育を介入としている先行研究の文献検討を追加で行っており、結果への期待や恐怖感や自己効力感が感染予防行動の関連因子であることを明らかにした。今後の研究の展開として、感染予防行動に関連する因子を参考にしながら患者教育を行うプログラム案を構成し、活用可能な教材の作成や学習支援システムの選定を進めていくことが必要である。
