项目摘要

特別養護老人ホーム(以下、特養)における看護実践能力向上のための教育プログラム実施に向けての準備を行った。実施方法については、受け入れ施設との相談の結果、いったんは対面で計画していたが、新型コロナウイルス感染症の流行により教育プログラムの実施の受延期の申し出があった。そこで、開始時期を延期したが、開催のめどがつかず、再度教育プログラムの実施方法を検討した。教育プログラムの実施方法をe-ラーニングとし、再度使用する教材を再検討し、現在準備を進めている。遠隔での教育プログラムとするため、研究参加者の人数を増加して行く方向である。また、特養における看護実践能力に看護師の個人因子や職場環境がどのように関連し、影響しているかを分析した。特養で勤務する看護師158名を対象に、自記式質問紙調査を実施した。職位、職場環境による特養における看護実践能力尺度得点の差を、Mann-WhitneyのU検定により算出した結果、職位、施設内・外研修への参加の有無、仕事量において有意に差がみられた。また、看護師としての経験年数、高齢者施設での経験年数、現施設での経験年数との関連について、Spearmanの順位相関係数を算出した結果、高齢者施設での経験年数、現施設での経験年数で有意な正の相関がみられた。さらに、特養における看護実践能力尺度を従属変数とし、個人因子等を独立変数としてステップワイズ法を用いた重回帰分析を行った結果、適合度は低かったが、職位、対人関係、現施設での経験年数、施設内研修への参加の有無に中程度から弱い影響力がみられた。この結果から、教育プログラム実施においても、看護職のおかれている環境にも考慮する必要性が示唆された。
