项目摘要

胆道閉鎖症をもつ思春期・青年期患者について、生活の乱れによる不要な肝移植治療への移行、生体肝移植への準備不足等の問題が顕在化し、患者のセルフケア能力を高めるために、看護職者が活用できる具体的なケアの指標の必要性が高まっている。本研究は、より具体的なケア指標の必要性の高まりに応え、先行研究で開発した『自己肝にて生存する思春期・青年期胆道閉鎖症患者が自ら療養生活を整えていくための患者と親へのケアガイドライン』について、有用性と継続的活用に向けた課題を検討することを目的としている。令和2年度・令和3年度は、「ケアガイドラインの有用性と活用課題に関する文献検討」を行い、「ケアガイドラインの活用に関する説明・検討会」・「臨床現場の看護専門職者によるケアガイドラインを用いた看護実践」の準備を整え、COVID-19感染により施設への立ち入りに制限があったものの、調査施設の倫理審査の承認を得て、調査施設内での調査準備に取り掛かった。最終年度である令和4年度は、すべてのケア実施者への「ケアガイドラインの活用に関する説明・検討会」を完了した。説明・検討会の中で、疾患に対する患者の知識不足、介入のきっかけづくりの難しさ、多忙な業務が障害となっていることが明らかになった。ケア対象者である思春期・青年期患者に配布できる疾患説明パンフレットを作成し、ケア実施者と共同のもと、ガイドラインを活用した継続的なケアを施行している。
