项目摘要

日本人脳梗塞患者における生活の質（quality of life：QOL）の低下を明らかにした。QOLの各項目、「移動の程度」、「身の回りの管理」、「ふだんの活動」、「痛み/不快感」、「不安/ふさぎ込み」に関して、患者状態、危険因子、画像所見、神経症状等の全ての患者要因との関連性を網羅的に検討した。EQ-5D-5Lの各項目（1-5点）を目的変数として、機械学習手法として勾配ブースティング決定木（XGBoost）、多クラス分類（softmax関数）を用いて、全ての関連因子について探索した。関連の強さは変数重要度により評価した。変数重要度の上位にある因子のうち、いずれのQOL項目にも共通した因子は、患者状態の中では年齢、body mass index（BMI）、脈拍、神経症状の中では上肢麻痺、下肢麻痺であった。一方、各項目に特異的に強く関連している因子としては、「移動の程度」に対して延髄梗塞、運動失調、「身の回りの管理」に対して呼吸器合併症、「痛み/不快感」に対して感覚障害、「不安/ふさぎ込み」に対して視野障害が認められた。QOLのあらゆる項目「移動の程度」、「身の回りの管理」、「ふだんの活動」、「痛み/不快感」、「不安/ふさぎ込み」に対して、高齢、低体重、さらに上肢あるいは下肢の麻痺が関連していることから、これらの因子は潜在的にQOLに大きな影響を及ぼしている可能性がある。一方、運動失調、感覚障害、視野障害など、梗塞部位に特異的な症状もQOLの低下に関与している可能性が示された。これらの症候を低減、軽症化できれば、QOLの改善に寄与することができると考えられる。

The quality of life (QOL) of Japanese patients with infarction is low. QOL items,"degree of mobility,""management of physical recovery,""activity,""pain/discomfort,""anxiety/anxiety,""patient status, risk factors, image findings, neurological symptoms, etc., all of which are related to the patient's essential factors, are discussed. One side, ataxia, sensory impairment, visual impairment, infarction site-specific symptoms, QOL and the possibility of deterioration are shown. The symptoms are reduced, the symptoms are reduced, and the QOL is improved.
