项目摘要

本研究の目的は、介護保険制度における訪問介護サービスにおいて、ホームヘルパーが認知症高齢者に対して用いる戦略的な行為や思考を明らかにすることである。ホームヘルパーを対象とするインタビュー調査を行い，認知症の特性を考慮し、効果的・効率的に自立支援を行うための経験知を探索した．インタビュー調査の対象者は，いずれも経験年数10年以上で訪問介護サービスについて熟知しているサービス提供責任者だった．インタビューの逐語録をホームヘルパーが生活援助サービスで行う戦略的な行為や思考に関する24項目と照合し，共通する項目と，独自性の高い項目を検討した．その結果、22項目については認知症高齢者に対する戦略的な行為や思考として適当であり、2項目については関連性が見いだせなかった．また、個別性の高い認知症の症状や、自身の症状に不安を抱える高齢者への対応が意識されており、22項目のうち概念名称と定義の修正が必要な項目と、カテゴリーの再分類の可能性が示唆された．認知症高齢者への対応については，ホームヘルパー同士での情報共有を意識的に行うことで，様々な認知症高齢者のケースを知りそれぞれの経験知を高めていること，また，業務の効率化よりも高齢者へのケアを意図した傾向が強いこと等を考慮し，認知症高齢者に対するホームヘルパーの戦略的な行為や思考の構造を考察することが，研究を展開する上での課題として明らかとなった．
