项目摘要

本研究では、訪問看護師がホームヘルパーに対し、独居要介護高齢者の安全に関してどのような教育的連携を行っているのか、その方法、内容と、訪問看護師及びホームヘルパー双方の認識について、①参加観察とインタビュー調査から質的に明らかにした後、②①の結果を用いた質問紙調査にて例数を増やし、一般化を図った。①参加観察とインタビュー調査では、独居要介護高齢者5名を担当する訪問看護師4名とホームヘルパー5名に研究協力を得た結果、訪問看護師が捉える、訪問看護師とホームヘルパーとの教育的連携について6つのカテゴリーが、さらに、ホームヘルパーが捉える、訪問看護師とホームヘルパーとの教育的連携について5つのカテゴリーが抽出された。①の結果を用いて行った②質問紙調査では、A市内の訪問看護ステーションで勤務する訪問看護師59名（回収率52.2%）名と、同じくA市内の訪問介護事業所で勤務するホームヘルパー251名（回収率51.1%）から得られた回答を分析した。訪問看護師は、【ヘルパーへ提供する知識】として、想定される状況と緊急時の連絡方法、【ヘルパーとの教育的連携のために獲得している手段】として、連絡ノート等の活用による担当者同士の直接的コミュニケーション、【ヘルパーとの教育的連携を促進する態度】として、ヘルパーが基礎的医療知識を持つことを重視していた。これらは、訪問看護経験年数が8年以上か、常勤である方がより意識している傾向がみられた。ホームヘルパーは、【介護専門職として獲得している技能】として、ヘルパーとしての身体状況の捉え、【訪問看護師との教育的連携のために獲得している手段】として、ケアマネジャーを通した連携、【訪問看護師との教育的連携を促進する態度】として、訪問看護師からの情報の利用を重視していた。これらは、介護福祉士免許を有しているか、常勤である方がより意識している傾向がみられた。
