项目摘要

がん化学療法では治療の奏効率と共に有害事象への対策が必要である。治療の奏効率を高めるためには治療が中断なく実施されることが望ましい。化学療法は年単位で実施されることが多く、長い闘病期間の中でいかに有害事象をコントロールできるかが重要である。現在は様々な状況から入院期間が短縮されている。化学療法もかつては治療期間中入院して実施されていたが、現在は薬剤の開発や治療法の変化により外来や在宅に移行している。これによって患者はある程度自分らしい環境の中で療養期間を過ごすことができるようになり、QOLの向上にもつながっている。一方で日常生活という医療者が身近にいない環境で、患者自身が様々な有害事象への対応や症状マネジメントを行う必要性に迫られている。従って患者のQOLを維持・向上させるためにも看護師は治療決定後速やかに効果的な指導を行い、患者のセルフケアやマネジメント能力を高める必要がある。有害事象の中で皮膚や口腔粘膜への有害事象は直接生命予後の関わることが少ないため、これまであまり注目されてこなかった。しかし、皮膚や粘膜は身体を守る命の袋であり、人体最大の臓器でもある。さらに皮膚は人目にさらされ社会生活を行っていくうえで個人のボディーイメージや尊厳に深くかかわっている。これまで我々は婦人科がんで化学療法を受けた患者の皮膚の生理機能を測定し、化学療法による皮膚バリア機能の低下について観察・検討した。今回の目的は生理機能の測定項目を増やし、加えて口腔粘膜のダメージを検証する。これらにより皮膚バリア機能の低下や口腔粘膜の有害事象を低減するにはどのようなケアが効果的であるかを検討する。それによって患者自身が日常生活の中でどのようなセルフケアをどのようにマネジメントしていけばよいか、そのために看護師はどのような指導を行えばよいか、患者のQOL維持のための方略を検討する。

The aim of this study is to increase the physiological function of oral mucosa. This is the reason why the skin function is reduced and the harmful effects of oral mucosa are reduced. For example, the patient himself or herself may be involved in the daily life of the patient, and the caregiver may be involved in the discussion of the patient's QOL maintenance strategy.
