医療の進歩により小児期発症の疾患をもつ患者の多くが成人期を迎えるようになった。成人への移行期における重要な支援のひとつに子どもの自立支援があるが、成人移行がスムーズにいかないという報告は少なくない。その要因の一つが親の子どもへの過保護的なかかわりだと指摘されているため、我々は子どものヘルスリテラシー(以下、HL)に見合った保護者の関わり、またHLの獲得を促進する保護者の関わりが必要であると考えている。今年度は支援の一助を得るため、小児慢性疾患患者の保護者が子どものHLをどのようにとらえているのか、移行支援に関する準備状況を明らかにするために実施した質問紙調査の分析を行った。10歳から19歳の小児慢性疾患患者の保護者179人のうち約60%の保護者が、子どもの病気と治療について子どもと一緒に話し合っており、子どもたちが小児科から成人科に移るときがいずれ来るだろうと、移行を受け入れていた。保護者による成人移行支援実施率は全般的に低いものの、子どもの年齢と保護者による支援の実施には正の相関があり、子どもの年齢とともに支援が進んでいるという強みも見出されている。一方、移行に導けるような情報収集や、子ども一人で外来診療をさせることなどの支援を実施できている親は20％未満であった。さらに、「子どもに自分の病状や治療上の健康状態を記録することの勧め」「経済的支援に関する情報収集」「子どもが成人した後の公的支援や医療保険の利用」などは、がんや血液疾患を持つ子どもの親よりも糖尿病や心血管疾患を持つ子どもの親の方が積極的に実施しているという、疾患による差異も明らかとなった。これらの結果から、子どもの移行をサポートする親の準備は、子どもの疾患によって異なっており、その要因を確認する必要があると思われる。また、親の準備状況は十分には進んでおらず、移行準備に関連する親への介入の必要性が示唆された。
