项目摘要

認知症対応型共同生活介護事業所 (認知症高齢者グループホーム) は、認知症に特化したケア拠点として社会的な期待を集めている。一方、認知症高齢者グループホームは、入居者の医療ニーズ高度化等、様々な問題を抱えている。認知症高齢者は衰弱のスピードが速いと言われており、入居者の身体機能の低下や体調の変化に対応した健康管理を行うには、介護職と看護職の円滑なコミュニケーションと協働が不可欠である。本研究の目的は、認知症高齢者グループホームにおけるフィジカルアセスメントと情報共有に関する実態を明らかにし、看護職・介護職の協働を目指した情報共有ツールの考案・開発を行うことである。研究の第一段階として、国内外の先行研究について文献的考察を行い、第二段階の研究として、認知症高齢者グループホームに勤務する看護職と介護職を対象とした半構造化インタビューを実施した。この結果、看護職・介護職の双方が、入居者の健康状態の情報を共有し、協働することの難しさを感じる一方で、共通の記録用紙や情報を橋渡しをする役割を担う等の工夫、お互いに気兼ねなく発言し、他者を尊重する組織風土の存在が示唆された。これらは組織の心理的安全性を示す特徴であり、認知症高齢者グループホームにおける介護と看護の協働にも不可欠な要素と考えられる。さらに、介護者側は自身の捉えた違和感だけでなく、血圧測定等のバイタルサイン、もしくは客観的測定値を看護職に伝え、看護職側も報告された数値と違和感について共にアセスメントを行い、対応する状況も示された。これらの知見を踏まえ、調査研究を実施する予定であったが、COVID-19の感染が未だ収束しておらず、研究計画の推進に支障が生じた。研究協力者らと協議し、専門分野の研究者よりスーパーバイズを受ける等、研究計画の調整を重ね、2023年度に改めて調査研究を実施してゆく予定である。
