(1)皮膚筋炎／多発性筋炎（DM/PM）に特徴的な骨格筋MRI所見と病理組織所見の関連を調べるために、特発性炎症性筋疾患（IIMs）が疑われる患者を対象として上腕または大腿の骨格筋MRIを実施した。MRI（STIRまたはガドリニウム造影T1強調像）で高信号を認めた部位において、局所麻酔下にen bloc筋生検を実施し、パラフィン切片および凍結切片を作製した。HE染色、 Masson’s trichrome染色、 elastic-van Gieson染色などを行い炎症細胞浸潤の局在を評価した。MRI所見は水平断像において、筋生検を実施した部位での高信号を呈する組織（皮下脂肪組織、筋膜、筋）および筋内での高信号のパターン（蜂巣状、霧状など）を評価した。その結果、炎症細胞浸潤と骨格筋MRI所見には関連性が示唆された。(2)各筋炎特異的自己抗体/筋炎関連自己抗体（MSAs/MAAs）別の骨格筋MRI所見の相違を解析したところ、MSAs/MAAsの種類によって骨格筋MRI所見に相違があることが示唆された。(3)骨格筋MRI所見とDM/PMの予後との関連について解析を行なった。悪性腫瘍や急速進行性間質性肺炎の併発は患者の死亡に直接かかわる事象であるが、これらを有する患者群において、特徴的なMRI所見が見出された。以上の結果から、骨格筋MRI所見は、IIMsの病理組織所見やMSAs/MAAsと関連することが示唆された。さらに、骨格筋MRI所見はDM/PMの予後とも関連する可能性が示唆された。これらの知見は、今後のIIMsの診断や治療において役立つことが期待される。
