【背景】近年、優れた抗腫瘍効果を期待できる分子標的薬などが開発され、治療精度が向上する一方で、腫瘍が急速に崩壊することによって起きる腫瘍崩壊症候群が従来以上に問題となっている。腫瘍崩壊症候群が生じた結果、急性腎不全が発症し一時的に血液透析が必要となり、原疾患の治療を遅延せざるを得ない状況も生じる。2016年5月にフェブキソスタットの適応に、がん化学療法に伴う高尿酸血症が追加された。腫瘍崩壊症候群発現予防の為には、迅速にUAを減少させる必要があるとの理由で、腫瘍崩壊症候群発現に対してはこれまでよりも高用量の60mgで承認された。【目的】本研究の目的は、フェブキソスタット20mgと60mgの腫瘍崩壊症候群発現予防効果を比較するとともに、尿酸変化量および腫瘍崩壊症候群発現の関連因子を抽出することである。【方法】2011年1月から2016年12月に東北大学病院における化学療法導入前に、フェブキソスタットを使用した悪性腫瘍患者について、フェブキソスタットの用量、性別、年齢、腎機能、癌種、使用抗がん剤、その他の使用薬剤、血液・尿検査値などの情報を収集した。尿酸変化量および腫瘍崩壊症候群発現の有無を従属変数とする重回帰分析および多変量ロジスティック回帰分析によって、尿酸変化量および腫瘍崩壊症候群発現の有無との関連因子を抽出した。【結果】本調査の結果、他の補正項目を考慮しても、尿酸変化量とフェブキソスタットの用量との間に正の相関が認められ、腫瘍崩壊症候群発現の関連因子としては、フェブキソスタットの用量低値と癌種として血液癌であることが抽出された。今後、過去に指摘されている腫瘍崩壊症候群発現の関連因子に加えて、本調査から得られた関連因子を考慮することによって、有効な腫瘍崩壊症候群発現予防方法を検討していきたい。

Background: In recent years, the development of molecular targets and the improvement of treatment accuracy have been expected for excellent anti-tumor effect. Acute renal insufficiency may occur as a result of acute renal failure, and hemodialysis may be necessary to treat the underlying condition. May, 2016: Chemotherapy with hyperuricemia was added. The reason for preventing the occurrence of tumor-related diseases is to reduce UA rapidly. The high dosage of 60mg is recognized. [Objective] The aim of this study was to compare the effects of 20mg and 60mg on the development of tumor-associated syndrome. [Methods] From January 2011 to December 2016, Tohoku University Hospital collected information on the use, dosage, gender, age, renal function, cancer type, anti-cancer agent used, other drugs used, blood and urine test values of patients with malignant tumors before chemotherapy introduction. Uric acid levels and the presence or absence of tumor-associated factors in the development of tumor-associated syndrome were extracted from multiple regression analyses. [Results] The results of this study, other correction items were considered, the amount of uric acid and the dosage of uric acid were positively correlated with each other, and the correlation factors for the occurrence of tumor collapse syndrome were determined. In the future, the correlation factors for the development of tumor-related collapse syndrome in the past have been added. The correlation factors obtained in this study have been considered. Methods for preventing the development of tumor-related collapse syndrome have been discussed.
