研究目的:近年、健康志向の高まりから健康食品(サプリメントも含む)の利用が増加してきている。しかしながら、副作用などに対する認識は非常に低いのが現状である。健康食品であっても副作用や相互作用が生じる可能性は十分あり、特に合併症を有する場合には、合併症の悪化や併用している医薬品との相互作用に対しても注意する必要がある。最近では、ある種の抗がん剤の中では添付文書中にビタミン剤や健康食品との相互作用に対する注意が記載されたものもある。また、併用により抗がん剤の作用増強や血中濃度減少なども報告されている。一方で、健康食品の相談室を設置し、内容の分析を実施している大学病院薬剤部があるという報告もある。今後、薬剤師も薬剤管理指導業務などで注意すべき分野であるが、科学的根拠に基づく情報が乏しく適正な指導が困難であるのが現状である。そこで今回、当院においてがん化学療法目的で入院中の患者を対象に、健康食品の摂取状況について調査検討を行った。研究成果:調査対象は薬剤管理指導患者を無作為に抽出した200名のうち約20%が健康食品を摂取していた。摂取している健康食品の内訳は、プロポリス、ミネラルおよびビタミン製剤など、直接的に制がん作用を期待したり、免疫力を高め間接的にがん抑制効果を期待すると思われる健康食品が上位を占める結果であった。健康食品を使用している患者の検査値や副作用歴について調査したが健康食品によると思われる副作用症状は認められなかった。また、健康食品と相互作用の報告がある薬剤との併用例は今回の調査において特に認められなかった。一方、免疫賦活物質についての情報たおいて、動物実験による抗腫瘍効果を証明する報告はあるが、臨床的に有用性を評価できる報告は少ない。例えば、アガリスクによる肝機能障害や発ガン誘発性の促進、大豆イソフラボンによる乳がん細胞増殖促進、β-カロチンによる発がん促進作用など、症状を悪化させる報告も多数存在する。このような情報と今回の調査結果より副作用回避のための健康食品データベースを構築した。今後、調査対象をさらに拡大し、本院における健康食品の現状について継続的に調査検討することにより、薬剤管理指導業務に応用するだけでなく、健康食品の適正使用の推進および相互作用による重篤な副作用発生を未然に防止することにより、質の高い患者サービス(文書による保健機能食品の情報提供など)が実践できると考える。
