项目摘要

補聴器装用効果の評価のためにVirtual Phoneを用いた疑似音場検査システムを開発した。その基礎的データとしてVirtual Phoneの音響特性をKEMARを用いて測定し、さらに正常者に対しSRT検査を施行しその基準値を求め、通常用いられているTDH-49受話器を用いた結果と比較した。その結果より、本疑似音場検査システムを補聴器装用効果の評価のための検査法として使用が可能なことを確認し、校正に必要なデータを得た。現在さらに、従来用いられている語表に加えてより選別性の高い語表を考案し、同検査システムを用いて補聴効果を検討する準備を進めている。一方、当科補聴器外来でリハビリテーションプログラムを受けた症例、補聴器を常時使用している症例、補聴器をほとんど使用していない症例の各群について、経時的な聴覚コミュニケーション能力の変化を自動語音検査装置(CASA)を用いた語音明瞭度検査、自己評定によるHHIEにて評価した。その結果、補聴開始後1年の時点では、リハビリテーションを受けた症例、補聴器を常時使用している症例において自覚的評価による聴覚コミュニケーションがより改善していることが認められた。裸耳および補聴時の最適語音明瞭度もそれら2群においてよりよい傾向が認められたが、いまだ症例数が十分ではなく引き続き検討を続けている。さらに左右対称性の難聴者で補聴器の片耳装用を行っている症例について、補聴側と非補聴側耳とで、裸耳の最高語音明瞭度およびそれをもたらす語音レベルについて経時的な変化があるかどうか検討している。
