フィリピン共和国における公的医療保険制度は、1969年に成立したPhilippine Medical Care Actに基づくPhilippine Medical Care Plan(Medicare)に始まる。同法は、被雇用を対象とするpartIおよび、その他の国民(自営業者、貧困層等)を対象とするpartIIからなるが、後者の運営は実質的にはなされてこなかった。partIの給付対象サービス、給付条件は、旧宗主国米国のMedicare制度を参考にしている。保険料は、給与の2.5%相当額を労使で折半し、給付率は33%であった(1989年)。民間企業の勤務者を対象にしたThe Social Security System(SSS)と、政府職員を対象にしたThe Government Service Insurance System(GSIS)がMedicareの中心的な組織となっているが、1989年の調査では対象労働人口の20%しか加入していない。1995年に施行されたNational Health Insurance Actは、National Health Insurance Cooperationを創設し、すべての国民に対する健康保険を一本化した国家医療保険制度を15年以内に成立することをめざした、発展途上国としては画期的な制度である。しかし、同制度には以下のような問題がある。1)人口の半数を占め、安定した現金収入をもたない零細農民・漁民をも含めた全国民の加入を目指しているが、運営に必要な財源の確保に問題がある(特にinformal sectorの就労者の保険料徴収)、2)National Health Insurance Cooperationは、最大の保険組合であるSSS、GSISの基金を引き継ぐが、それ以外の保険組合の基金は独立のままであり、SSS GSISの加入者にとって不公平である。国家医療保険制度の成立と運営の継続のためには、上記1)の問題への対策が不可欠であり、保険料の決定方法、徴収方法等、運営の具体的な方策を検討する必要がある。他の途上国に比して、フィリピンでは保健分野のNPOの活動が活発であり、Health Maintenance Organizationや、住民による共済組合がさまざまな組織・地域で運営されている。公的保険の導入のみに頼ることなく、これら民間組織のサービスも組み合わせることによって、住民の便益を最大にすることが当面必要である。

The Philippine Medical Care Act was established in 1969 as the basis for the Philippine Medical Care Plan(Medicare). The same law applies to employees, employees, and other citizens (self-employed, poor, etc.). The establishment and operation of the national medical insurance system, the problem of 1) above, the countermeasures, the determination method of the insurance material, the characteristics method, etc., the specific operation of the policy are necessary to discuss. The Health Maintenance Organization and the Community Management Organization are the key components of NPO activities in China. The introduction of public protection is necessary, and the participation of non-governmental organizations is necessary to maximize the benefits of residents.
