平成17年度は、1990年代において「新契約加入保証」「契約更新保証」「料率規制」などを内容(改革の手段)とする州政府の医療保険制度改革(小雇用主医療保険の改革)が、(1)小雇用主の「保険利用可能性」と「保険料負担可能性」に与えた影響と効果、(2)アメリカに特徴的な無保険者問題に対して有する意義と問題点について研究し、以下の点が明らかとなった。(1)保険利用可能性と保険料負担可能性に与えた影響と効果小雇用主(とくにリスクの高い小雇用主)は、「新契約加入保証」と「契約更新保証」によって「保険利用可能性」が改善されている。しかし、保険者がそれぞれの小雇用主のリスクに厳密に基づいて料率を設定すると、リスクの高い小雇用主の料率はかなり高くなる。この点において「料率規制」はもっとも重要な改革の手段であり、それによってリスクの高い小雇用主の「保険料負担可能性」は改善されている。しかし他方で、リスクの低い小雇用主は、料率が上昇するために「保険料負担可能性」が低下しうる。この点に関して、1990年代に料率が比較的安いケアマネジドケア・プランを選択する小雇用主が増加している。つまり、一方で被用者と扶養家族に医療保険を提供するリスクの高い小雇用主が増加し、他方でリスクの低い小雇用主は医療保険を提供しなくなったわけではないという点において、「料率規制」は一定の効果をもたらしたといえる。(2)無保険者問題に対して有する意義と問題点上記のように、「料率規制」は「保険料負担可能性」の改善に一定の効果をもたらしている。しかし、「料率規制」は小雇用主間の料率の格差とそれぞれの小雇用主の料率の変動を抑制するものであり、料率の絶対額を全体的に抑制するものではない。したがって、「保険料負担可能性」を改善し、無保険者問題を解消していくためには、小雇用主医療保険の改革で導入された「料率規制」だけでは不十分であり、医療費を抑制する「マネジドケア」も有効に機能する必要がある。
