昨年度の途中報告で示した3症例のうち,1症例(8歳女児)の検体から細胞変性効果が認められ,単純ヘルペスウイルスの存在が強く示唆された。他の2例は盲継代を繰り返していたが,18回と20回の盲継代でも徴候がまったく認められなかったので断念いたしました。本年度も昨年同様に疱疹性口腔粘膜疾患を集めているが,やはり症例の絶対数が少なくサンプリングに供した症例は7症例です(1999年2月15日)。その内訳は細胞変性効果が認められたもの2例,盲継代中のもの2例,検出を断念したもの3例です。細胞変性効果が認められた3例についてはそのウイルスを増殖させ冷凍保存するいっぽう,その定量(プラーク形成単位)と同定(免疫染色法)をおこなった。定量は希釈系列と計算式から比較的容易であったが,その同定は手技的未熟さからか現時点では不完全である。細胞変性効果の特徴や後記するアシクロビルに対する感受性から単純ヘルペスウイルスI型と確信しているがクリアーしなければならない問題として残っている。ウイルスは多めに保存してあるので必ず同定できるものと考えています。主題である臨床分離ウイルスの抗ウイルス剤アシクロビルに対する感受性試験は当初の計画通りプラークリダクションアッセイ法を使用した。アシクロビル無添加の対照群に対する50%阻止率を示す濃度IC_<50>は0.06,0.12,0.12μg/mlであった。また,IC_<80>はそれぞれ0.5,1.0,0.5μg/mlであった。今後も症例を集め例数およびデータを増やしていかなければなりません。当面は10症例を目標に継続していく予定です。
