平成10年度の研究概要は以下の通りである。出産後の褥婦に分娩体験の所感、産痛体験の所感などの聞き取り調査を平成9年度10名、平成10年度6名に行いその結果を基に出産前教育における産痛教育の検討を行った。1. インタビューの結果よりインタビューの結果以下のことが明らかになった。褥婦は(1)産痛を個人的体験として捉えている。(2)出産前の産痛へのイメージが実際体験した産痛の程度の認識に影響している。(3)産痛へのイメージや産痛への対処行動はこれまでの個人の経験が大きく影響している。(4)産痛の程度よりも産痛時の対処法についての知識を求めている。(5)分娩中の助産婦の痛みへの共感や痛みの存在の肯定により、自己肯定感や精神的安寧感を強め、産痛や分娩へ向かう姿勢を強化させている。(6)産痛を通して児への愛着や自分自身への自信、女性への畏敬の念などを感じ産痛へ意義を見いだしてる。(7)今回のインタビューにより自己の分娩・産痛体験を振り返り整理することが出来ていた。2. 産痛教育のあり方について現在行われている出産前教育は集団での指導が主であるが、褥婦は、産痛を個人的体験として捉え、他者と比較できるものではないとしていること、また、それまでの個人の体験や生活状況によって産痛へのイメージが異なることから、産痛教育は画一的な集団での指導ではなく、個々人に対応した指導が必要性である。そして、産痛そのものを伝えるだけでなく、個人に適した産痛への対処法を示すことが大切である。さらに、出産前だけでなく、実際の分娩の際にも、助産婦は産婦の感じている産痛やその体験を共感的にありのままに受け止めることにより産婦を支えることが出来よう。インタビューが産婦の体験を再構成し、産痛の意義の発見に有効であったことから、分娩後に産痛体験を含めた分娩体験のレビューの必要性も認められた。よって、産痛教育は、画一的に集団で行うのではなく、個々人の特性や痛みへの認識を理解した上で、痛みを伝え対処法を指導することまた、分娩中、分娩後の助産婦の関わりが褥婦への産痛体験の認識に重要な影響を及ぼすことを認識し産痛教育に当たることが大切であろう。
