成人性歯周炎患者におけるIL-1α、IL-1β発現調節領域遺伝子の遺伝的多型性を解析し、同遺伝子型が成人性歯周炎の重篤度、罹患性及ぼす影響の検討を計画した。大阪大学歯学部附属病院歯周病診療室を受診し、重篤な全心疾患を有さず、年齢が35歳以上55歳以下の非喫煙患者から、大阪大学歯学部倫理委員会の承認のもと、実験参加の承諾が得られた者を被験者として選択した。初診時全顎デンタルレントゲン写真を画像解折した結果をもとに、被験者を健常群と成人性歯周炎患者群とに分けた。即ち、24歯以上の残存歯を有し、かつレントゲン的に3mm以上の骨吸収を示す部位が2部位以下の被験者を健常群とした。中等度以上の成人性歯周炎患者群は、12歯以上の残存歯を有し、骨吸収の全顎の平均値が3mm以上、または3mm以上の骨吸収を示す歯牙が少なくとも10歯以上存在し、かつ各1/4顎に3mm以上の骨吸収を示す部位が2部以上存在することを基準に選択した。その結果、平成10年度の被験者から、健常群18名(男性18名)、患者群41名(男性8名、女性33名)の参加を得て解析を開始した。被験者の末梢血細胞由来ゲノムDNAを鋳型として、IL-1α及ひIL-1βの遺伝子多型性が存在するプロモーター領域を本研究費で購入したコンピューター装置を用いて設計したPCRプライマーにて増幅し、両遺伝子の多型性固有のDNA配列を認識する制限酵素で切断した。DNAの泳動パターンからIL-1αとIL-1βの両者を高発現する遺伝子型を有する被験者をIL-1陽性と判定した。その結果、健常群18名中からは0名、患者群41名中1名がIL-1陽性を示した。また、上記分類以外の軽度歯周病患者47名中4名がIL-1陽性であった。白人をもとにした解析結果に比較してIL-1陽性率が著しく低いため、今後更に検体数を増やして解析を進めることが望まれる。

Results: 18 healthy subjects (18 males) and 41 patients (8 males and 33 females) participated in the analysis. The gene polymorphism of IL-1α and IL-1β in the peripheral blood cells of the patients was studied at the expense of PCR amplification and restriction enzyme cleavage. DNA movementsIL-1α and IL-1β are highly expressed, and IL-1 is positive for those with DNA movementsIL-1 β and IL-1 β. Results: 0 out of 18 healthy subjects and 1 out of 41 patients were IL-1 positive. IL-1 was positive in 4 out of 47 patients with severe dental diseases other than those listed above. The positive rate of IL-1 in white people was lower than that in white people, and the positive rate of IL-1 in white people was higher than that in white people.
