適応障害における個体側の要因と環境因との関係を明らかにするために、I.今日までに蓄積していた適応障害症例に対する疫学調査、II.新たに発生した適応障害症例に対する症状の経過や予後に関する調査を立案し、遂行している。平成11年度は主にIの調査が主体で、IIについては症例を蓄積中である。蓄積していた病棟リエゾンにおける適応障害411例中詳細な調査が可能であった363例について疫学的な検討を行った結果、(1)年齢分布は男女共2峰性であるが、ピーク年齢などには男女間で異なり、男女共20歳台に最初のピークがあるが、男性では50歳台に、女性では40歳台に次のピークがあること。(2)身体状況としては「自殺企図・自傷行為」が最多で、次いで「癌」が多く、両者で67%を占めていたこと。(3)「自殺企図・自傷行為」、「癌」のそれぞれで年齢分布に男女間の差があり、「自殺企図・自傷行為」では男女共20歳台にピークがあるが、男性では50歳台が多いのに対し、女性では10および40歳台が多いこと。また、「癌」では男性が50、60歳台に多いが女性は40、70歳台に多いこと。(4)同居家族の構成は男女間で差があり、男性は3、4人が多いが女性は2、3、4人が多く同症例数であること、男女共「本人・配偶者・子」という構成が多いが、次いで多いのは男性では「本人一人暮し」、女性では「本人・配偶者」であること。(5)職業は男性では給与所得者が最も多く、女性では専業主婦、給与取得者の順であること、などが明らかとなった。適応障害症例は、「自殺企図・自傷行為」のように適応障害を起こして入院する症例と「癌」のように入院中に適応障害が生じる症例が存在し、実際に両者だけで全体の2/3を占めていることがわかった。個体側の要因として年齢・性別を中心に他の要因を比較したが、それぞれで違いがあることもわかり、今後さらにそれらと環境因を明らかにする研究を続けていく予定である。
