项目摘要

満田久敏は、昭和17年に臨床遺伝研究から分裂病性精神病を定型分裂病と非定型精神病に類別し、両疾患が臨床的にも遺伝的にも独立した疾患であると報告した。すなわち、定型分裂病は慢性に経過して人格変化を来たすが、非定型精神病は急性に発症するもののほぼ寛解する。我々は、満田の非定型精神病概念を再検討するために、定型分裂病30例、非定型精神病30例、正常対照群50例で、横S字図形を用いた探索眼球運動を比較した。分裂病の探索眼球運動において、記銘課題時に注視点の動く範囲が狭く、また、比較照合課題時の念押しの質問に対する反応的な注視点の動き(反応的探索スコア)が少ないといった所見が報告されている。本研究の結果、記銘課題では、正常対照群と非定型精神病群はほぼ同じ値を示し、定型分裂病群のみ低い値を示した。一方、比較照合課題における反応的探索スコアでは、正常対照群が最も高い値を、定型分裂病群が最も低い値を示し、非定型精神病群はその中間の値を示した。さらに、精神生理学的相違から分裂病性精神病の異種性を検討するために、定型分裂病20例、非定型精神病23例、正常対照群23例の3群で、事象関連電位P300と横S字型図形による探索眼球運動を測定し、多変量解析を施行した。その結果、定型分裂病の約半数が、反応的探索スコアの最も低い群に分類したが、P300振幅の最も低い群と一致しなかった。また、定型分裂病と正常対照群の大部分は異なる群に分布した。一方、非定型精神病は、主に高振幅のP300と不安・抑うつ症状を伴う群に分布するものの、P300の低振幅を示す群にも認められた。以上の結果は、定型分裂病と非定型精神病とが異なる病態を示す疾患である可能性を示唆している。

In the 17th year of 2007, Hisatoshi and Showa conducted clinical research on schizophrenia, schizophrenia and non-schizophrenia. The results of this study are summarized as follows: 1. The differences between normal and atypical psychiatric groups are significant, and the differences between atypical and atypical psychiatric groups are significant. For one thing, compare the research results of the research on the subject, and show the highest middle value of the normal contrast group, the lowest middle value of the typed schizophrenia group, and the middle value of the atypical psychosis group. In addition, the psychophysiological correlation between schizophrenia and heterogeneities was studied in 20 cases of type schizophrenia, 23 cases of non-type psychosis and 23 cases of normal control group. The event-related potential P300 was measured in a horizontal S shape. About half of the results, typing and splitting diseases, and the lowest group of P300 amplitudes are consistent with each other. The distribution of most of the normal groups of chromosomes is different. A party, non-stereotypical psychosis, the main high amplitude of the P300, suppression of symptoms associated with the group distribution, P300, low amplitude of the group recognition These results indicate the likelihood of a disorder ranging from stereotypical schizophrenia to atypical psychosis.
