项目摘要

特別養護老人ホームに入所中の高齢者にアクティビティケアとして園芸療法を実施し,対象者の反応を観察した。その中から,対象者への効果,ケア施設での効果的な実施方法が明らかになった。今回実施した園芸療法は,1人が一鉢を育てる方法で,6〜8人を対象とした月1回の集団活動と毎日個別活動を実施した。集団活動は,植物や食物を利用した回想と育てている植物のメンテナンスを実施し,個別活動では,水遣りなどの世話を実施した。対象者の効果としては,毎日植物の世話をスタッフと共に実施することにより,スタッフと向き合い共に植物の成長を楽しみ,生活の中の楽しみが増えた。さらに,植物がスタッフや他の入所者との共通の話題となり,コミュニケーションが促進された。効果判定のために使用した痴呆性老人の生活の質尺度やMDS-Rapsでは,有意な変化は認められなかったが,対象者の特徴を考慮すると,変化のないことも一定の効果であると考えられた。実施方法としては,今回実施した特別養護老人ホームでは,リハビリの専門スタッフが常勤しておらず、集団活動が短い周期でできなかった。しかし、個別活動を併用することにより園芸療法の効果が増すことがわかった。集団活動は,仲間作りの機会となるとともに,他者の植物と自分の育てている植物を比較する機会ともなり,育てている植物に愛着を持つきっかけとなっていた。個別活動では,毎日植物の世話をすることで,毎日の日課ができ,生活のリズムを作り出すことにつながるのではないかと考えられた。実施上の問題点としては,痴呆が重度に進んだ高齢者や生活環境の変化を極端に嫌う高齢者には,今回実施した1人一鉢を育てる方法では難しいことがわかった。また,植物の選択では,高齢者になじみがあり成長の変化が大きいものが好まれ効果が高いと思われた。
