1)患者のH.pylori感染の検索本年度は、主にアトピー性皮膚炎における抗体の検索を行った。抗H.pylori-IgG抗体陽性者は,56名中16名(29%)であった。年代別では,7〜19歳が1名(6%),20〜29歳が8名(25%),30歳以上が11名(53%)であった。重症度別では,軽症が3名(20%),中等症が5名(25%),重症が8名(38%)であった。抗H.pylori-IgE抗体陽性者は,56名中10名(18%)であり,年代別では,7〜19歳が3名(18%),20〜29歳が3名(15%),30歳以上が4名(21%)であった。重症度別では,軽症が1名(7%),中等症が3名(15%),重症が6名(29%)であった。IgE型抗体の結果を解析したところ、IgG抗体,好酸球,LDHとの間には,相関関係はみられなかった。IgE型抗体陽性群は,陰性群に比べ,カンジダ,ピチロスポルム,トリコフィトンのRAST値がいずれも有意に,高く、アトピー性皮膚炎の重症患者は,軽症患者に比べて,抗H.pylori-IgE抗体価が有意に,高かった。2)免疫組織学的研究は、いまだ2-3例の検討が終了した段階で、最終的な結論は得られていない。今後、症例の蓄積に最大限の努力をくわえるつもりである。3)消化器悪性腫瘍との相関消化器症状を示さない患者で、標準的治療抵抗性の掻痒患者82名のうち5%に胃癌が発見された。この割合は、同時期に消化器症状を訴えて内科を受診した患者での胃癌の頻度とほぼ同様であった。年齢をマッチさせた健康診断での胃癌発見率0.1%前後であることから、極めて重要な発見となった。以上から、皮膚と消化器の間にには、病因的に有意な相関関係が存在するものと考えられた。今後、コントロール試験を行い更に精度の高い研究にしていく予定である。

1)The detection of H.pylori infection in patients is ongoing this year. 16 (29%) out of 56 cases were positive for anti-H.pylori-IgG antibody. Age categories: 7 to 19 years old 1 (6%), 20 to 29 years old 8 (25%), 30 years old and above 11 (53%) 3 cases (20%) were severe, 5 cases (25%) were moderate and 8 cases (38%) were severe. Anti-H.pylori-IgE antibodies were positive in 10 out of 56 (18%), 3 out of 7 to 19 (18%), 3 out of 20 to 29 (15%), and 4 out of 30 (21%). The severity of the disease was 7%(1 case), moderate 15%(3 cases) and severe 29%(6 cases). The results of IgE antibody analysis, IgG antibody, good acid ball,LDH, and the relationship between them. IgE antibody positive group, negative group 2)Immunohistologic studies were conducted in 2-3 cases and the results were conclusive. In the future, the accumulation of symptoms will be limited to the maximum effort. 3)Digestive ulcers and related digestive symptoms were reported in 82 patients with itching resistant to standard treatment, and gastric cancer was reported in 5% of patients. The frequency of gastric cancer is similar to that of digestive symptoms. The incidence rate of gastric cancer was 0.1%. The relationship between the above factors and the digestive organs exists. In the future, the test will be carried out with greater precision.
