今年度は、まず昨年度実施した予備的調査の分析を行い、特別養護老人ホーム(以下、特養)で提供されているショートステイ・ケアの現状と課題を明らかにした。特養は、介護老人保健施設や介護療養型医療施設に比べ、重度の痴呆性高齢者を受け入れており、利用中の事故防止や利用者間とのトラブル防止、またケア方法について家族と了解を得ること等に難しさを抱えていた。ケア課題としては、個別性を重視したケア、入所に伴う不適応(帰宅要求、暴力等)、在宅ケアの延長としての施設ケアのあり方等をあげていたが、利用後、ケア評価を実施している施設は1割に満たなかった。今後は、提供したケアが利用者、家族にどのように影響したのか利用後の生活状況をフォローし評価する取組みが必要である。以上の調査結果をふまえ、どのようなケアがショートステイ利用前〜利用中〜利用後に必要であるのか、実際に行われているケアを事例毎に観察・面接することによって探索した。対象は、介護が困難と予測される痴呆性高齢者3名とその家族、そして彼らをケアする施設職員である。事例1:75歳、女性。利用期間は平成14年10月22日〜10月31日。事例2:69歳、女性。利用期間は平成14年11月25日〜11月27日。事例3:80歳、男性。利用期間は平成15年1月24日〜1月27日。利用前・後に実施した本人と家族への訪問面接調査と利用開始3日間7:00〜19:00のタイムスタディおよび全利用期間中の本人と施設職員への面接調査により、3事例に提供されたケアを評価・分析した。その結果、ショートステイ・ケアの要点として、入所以来変化する利用者の反応を予測したケア、利用者と他の入所者との人間関係を調整するケア、安全の保障のみならず自立維持・改善をすすめるケア、家族とケア方法を共有しつつ家族の心身の健康を支えるケアが抽出された。
