项目摘要

昨年度報告したとおり、看護師の静脈内注射(以下、静注)の実態に関する調査を行った。調査期間は平成17年3月〜7月であり、調査用紙の配布は、全国の病院152施設に所属する教育担当者152名と看護スタッフ2713名に行った。教育担当者に対する調査内容は静注に関する院内教育の内容や看護学基礎教育への期待などであり、看護スタッフへの調査内容は看護師の静注の実施状況などであった。教育担当者を対象とした調査の結果、回収されたデータは112(回収率73.7%)であった。静注に関する院内教育を「行っている」のは74名(66.1%)、「行っていない」のは38名(33.9%)であった。現在の静注に関する院内教育への意見は、「十分役立つものである」のは1名(1.4%)であり、他の73名(98.6%)は「今後は教育内容の検討が望ましいあるいは検討が必要」と回答していた。看護学基礎教育における静注の教育内容への意見は、「現状のままでよい」「現状のままで対応できる」のは24名(21.6%)であり、87名(78.4%)は「できるだけ改善してほしい」「早急に改善してほしい」との回答だった。看護スタッフを対象とした調査の結果、回収されたデータは1602(回収率59.0%)であった。静注の実施状況は、準備から抜針に関して作成した31項目中21項目について80%以上の看護師が行っていた。静注に関する知識・技術の習得状況について自己評価の高い項目は、輸液ボトルの混注・輸液セットの接続、静脈針、留置針の穿刺・固定等であり、「十分に習得できている」のは60%を超えた。一方、自己評価が低い傾向にある項目は、抗がん剤など特殊薬剤の作用・副作用への理解であり、「十分に習得できている」のは10%代であった。今後は静脈内注射の教育の中で、薬理学等の基礎的知識の理解を深めるプログラムを立案する必要性が示唆された。
