项目摘要

2年計画で行った本研究では、本年の最終年度に、がん告知後の患者を対象に、初年度に開発した看護職によるコミュニケーションスキルを用いた看護支援法の有効性を、患者のQOL、心理状況、およびケア満足度について検討することを目的に無作為化比較試験を行った。対象をAがん検診機関にて消化器(胃・大腸・食道)がんの診断・告知を受けた者とし、ケアを提供する看護職を無作為に介入群4名、対照群4名に割付けてケアを提供した。介入内容は、コミュニケーションスキルSPIKESを用いた患者面接の実施を告知当日、1週間後、1ヶ月後の3回行った。対照群へは従来のケアを実施した。測定用具として、患者のQOLを測定するCQLI尺度、心理状況を測定するHADS、および看護職へのケア満足度をVASにて測定した。介入の評価時期は告知後1週間、1ヶ月、3ヶ月の計3回とした。介入効果の分析には繰返しの分散分析を用いた。本研究はAがん検診機関長の承認を受けて行い、また、研究対象者に対して研究内容を十分に説明した後参加同意を得て行った。結果として、対象数は64名(介入群31名、対照群33名)、対象の特徴は、平均年齢64.9±9.2歳、性別は男性34名(53%)、がんの部位は胃40名(63%)、がんの進行度は早期36名(56%)であった。介入効果については、CQLI、HAD-T、および看護職へのケア満足度の群間と時間の交互作用項に有意差が認められた。さらに、事後テストを行った結果、CQLIは告知後1ヶ月時点、HAD-Tは告知後3ヶ月時点、ケア満足度は告知後3ヶ月時点にそれぞれ介入群は対照群に比べて有意に良好であった。この結果から、看護職によるコミュニケーションスキルを用いた本ケア法は、がん告知後の患者のQOL、心理状況およびケア満足度を長期にわたって高めるのに有効であることが示された。今後は、本支援法の実践への普及が課題である。
